Esketamine / Spravato
SPRAVATO™, or Esketamine, is an innovative FDA-approved treatment that has relieved thousands of individuals suffering from treatment-resistant depression. What is SPRAVATO™?
SPRAVATO™ is an esketamine nasal spray that is used in combination with an oral antidepressant. SPRAVATO™ can alleviate the symptoms of treatment-resistant depression and Major Depressive Disorder (MDD). Spravato isn’t like other antidepressant treatments. Standard antidepressant medications boost levels of naturally formed chemicals in the brain like dopamine, norepinephrine, glutamate, and serotonin. SPRAVATO™ is prescribed in one of two dosages: 56mg or 84mg. During treatment, you will rest comfortably under the supervision of a certified SPRAVATO™ specialist for approximately 2 hours. SPRAVATO™ can trigger some short- lived side effects such as dizziness, dissociation, and sedation. These side effects usually happen right after taking SPRAVATO™ and go away the same day. Once the provider has released you, you will need a friend or family member to drive you home.
Standard SPRAVATO™ Treatment Plan:
Month 1: Two treatments per week
Month 2: One treatment per week
Month 3+: Continue weekly treatment Or Treatment once every 2 weeks

What is treatment Resistant Depression?
In the SPRAVATO™ clinical trials, treatment-resistant depression was diagnosed in adults who were struggling with major depressive disorder and had not responded adequately to at least two different antidepressants of adequate dose and duration in the current depressive episode.

How is Spravato nasal spray different than ketamine infusion?
SPRAVATO™ (esketamine) CIII nasal spray is a derivative of ketamine. SPRAVATO™ is an FDA-approved nasal spray for treatment-resistant depression; it is also approved to treat depressive symptoms in adults with major depressive disorder with suicidal thoughts or actions. Ketamine has not been approved by the FDA to treat depression. Both SPRAVATO™ and ketamine are Schedule III controlled substances under the US Controlled Substances Act.
What personal items can I bring to my treatment session?
You can bring personal items that make you feel comfortable which may include headphones, a music player, a tablet, crossword puzzles or a book to pass the time, and a blanket and pillow to help you relax.
What happens during the observation period after taking SPRAVATO™?
The observation period begins right after the treatment is administered. During this time patients will be allowed to rest comfortably and will be monitored by a healthcare provider for any side effects you may experience. You should expect your healthcare provider to ask you about the side effects and have your blood pressure taking periodically during the observation period.
What side effects could I experience on SPRAVATO™?
Other possible serious side effects include increased blood pressure, problems with thinking clearly, and bladder problems. The most common side effects include feeling disconnected from yourself, your thoughts, feelings, and things around you (dissociation); dizziness; nausea; feeling sleepy (sedation); spinning sensation; decreased feeling of sensitivity (numbness); feeling anxious; lack of energy; increased blood pressure; vomiting; feeling drunk; and feeling very happy or excited.
Serious side effects reported with SPRAVATO™ include sedation and dissociation, and there is a risk of abuse and misuse with SPRAVATO™. There is an increased risk of suicidal thoughts and behavior with SPRAVATO™. Call your healthcare provider right away if you have new or sudden changes in mood, behavior, thoughts, or feelings, or if you develop suicidal thoughts or behavior.
Because of the possibility of nausea and vomiting, you should avoid eating two hours before, and drinking liquids 30 minutes before, taking SPRAVATO™.
How quickly might I see improvements in depressive symptoms with Spravato for treatment-resistant depression?
In clinical studies, improvement of depressive symptoms was demonstrated at four weeks by SPRAVATO™ plus an oral antidepressant, compared with placebo nasal spray plus an oral antidepressant. Not all patients will respond to SPRAVATO™.
What should I tell my healthcare provider before starting Spravato?

Before starting, tell your healthcare provider about your full medication and depression treatment history, including if you have a history abusing prescription or street drugs, or a problem with alcohol. Do not take SPRAVATO™ if you have: a blood vessel disease (aneurysm), an abnormal connection between your blood vessels (arteriovenous malformation), a history of bleeding in the brain or if you are allergic to esketamine, ketamine or any of the ingredients in SPRAVATO™ . Be sure to tell your healthcare provider if you have heart, brain or liver problems; if you have psychosis; or if you are pregnant, planning to become pregnant, breastfeeding or planning to breastfeed. You should not take SPRAVATO™ if you are pregnant. You should not breastfeed if you are taking SPRAVATO™.
Tell your healthcare provider about all the medicines that you take, including prescription and over-the-counter medicines, vitamins and herbal supplements.
